Help with Defences.

I have no idea how I died I was clearing blood aqueducts just farming it for levels and currency and I killed a pack and I guess it dropped a on death effect that 2 shot me I remember the first hit taking half my hp then the second killing me with no time for reaction.


I'm wondering what could have done this I am Res capped, running determination, purity of elements for ailment immunity, and defiance banner.

I thought my defenses were solid can someone take a look and see if I was missing something with my build.

Anyone at all lol?
