can anyone help me figure out whats wrong?

So this is the first time I've built my character without a guide, My DPS seems very good aside from the occasional enemy that can tank most of my phys dps, but definitely suboptimal. But my main issue is that after regular t16 maps I often get evaporated in 1 hit, and I don't really know what I can do to improve my survivability from this point. I clear regular maps pretty fast, but when i try to go into simulacrum I get wiped by Kosis, and allot of these other bosses feel like my build is making them scarier than they should be. Anybody got any tips?

https://pobb.in/AdjuTJ--EDGD , did some small changes, you have very little health pool with no additional defensive layers, chancing endurance charges to match frenzy should help a lot, try to implement petrified blood so you don't just explode if it still keeps happening.
