Exalted Orb consumed used without adding a modifier to a Rare

The Exalted Orb's description says it can bring a Rare up to having 6 modifiers. I used it on a Rare weapon that had 5 modifiers already on it, and nothing happened. The Exalted Orb simply disappeared. It was consumed, but gave me no benefit. There are still only 5 modifiers on it.

Is the real limit 5 and the description that says 6 is wrong? If so, shouldn't that prevent me from using the Exalted Orb so that I don't waste it?

This is in POE 2, by the way. Sorry, I should have put that in the title.
Same thing has happened to me twice. I have found 2 exalts so far, and both times I used on a rare with 4 modifiers, and the rare stayed the same 4 modifiers and the exalt was consumed.
The same happened to me just now. Used exalted orb on body armor with 4 modifiers, orb disappeared but no new modifier added to the armor.
There is a chance that you have a double mod on your item and you can't see it normally when pressing alt, For instance....

There are some mods that add both life and evasion. (And is considered one mod, a prefix) Now when you ex it, you get a life mod, so the item gives more overall life, but still shows the same amount of mods.

There are other mods like rarity that can be both a prefix and a suffix, there's a chance you rolled that also resulting in a bigger rarity mod.

This happened to me a few days ago too. Surely if there is a double mod then we wouldn't be able to exalt it due to already having 6 mods?

That's not what he means. Here's an example:

Let's say you have a body armour with the following mods:

1) 15% evasion and 20 life (this is a hybrid mod meaning it counts as 1 mod but shows on 2 lines).

2) 40 fire resist

3) 20 cold resist

You would see:

15% evasion
20 life
40 fire resist
20 cold resist

So that's 3 mods but you will see it on 4 lines because the first mod shows up on 2 lines.

Now if you throw an exalt and let's say it hits 100 life as a new prefix mod, you will see this:

15% evasion
120 life
40 fire resist
20 cold resist

Still 4 lines but now you have 4 mods. The game simply combines like lines together.

To see mods separated by line, you need to hold ALT when looking at the item. It would then show you:

15% evasion
20 life
100 life
40 fire resist
20 cold resist

Wow, I didn't realize this could happen.

I have a rare evasion/es body armor that I was exalting and ended up with the body armor saying:
104% increased evasion and energy shield
+73 to maximum life

But when I alt-click on it, it's actually 3 mods:
76% increased evasion and energy shield <- T5
+45 to maximum life <- T4
28% increased evasion and energy shield <- T4 (hybrid)
+28 to maximum life
