Quest Item Not Dropping in Venom Crypt

I went into the Venom Crypt to do the quest but realized I hadn’t received the quest yet. I cleared the cave and noticed the quest item didn’t drop. After going back to town, I got the quest from the quest giver and went back to redo the objective, but the item still didn’t drop.
Now, my quest log shows the quest as completed, but it also says it’s unfinished. Now i can't get the 10% chaos resistance from the quest anymore. Último bump em 15 de fev de 2025 09:44:22

I have the same issue in act 3 normal diffuculty. It seems if you die when the mob who dropped the item its just gone and never comes back. I made like 5 other runs for The Venom Crypts but can`t get it. So i basicly can delete this char? Anyone got a solution?
