Omen of Ressurgance doesn't work

I've died 3 times with this in my inventory. I thought I was just getting one shot and never going into low life territory but I just saw my life dip to 5% and it still didn't go off, almost entirely empty health globe and no activation.

Did you right click to activate it. Not sure obvious but thats what you gotta do

Just wanted to add my experience, during a typical map boss fight, I have the omen activated and have seen it not go off twice now this week. Not sure what is going on because it use to work without any issue but something seems to have changed. After I died, I still have the active omen in my inventory. It is clearly not working anymore.

Same here. My first one worked, that was just yesterday. I didnt directly see it, but i barely survived one fight, and back in my hideout i noticed it was gone. Immediately bought new ones and died twice since, omen still active.

Even tho OP used it wrongly, came here to mention that it did not work for me. Yes it was activated, with the red border around them.

I was frozen, and died even though my HP reached Low Life (30%). Not sure if being frozen means it would not get activated tho.
