I figured it out - Flask use Resets Combo points

I actually figured out what's going on with the below bug.

Using a flask resets combo points and Bell disappears.

I first encountered the bug when I did a passive point respec.

Later I did another respec and the bug went away.

I just now did another respec and the bug is back. When the bug is active on a character there is a "ting" sound that occurs when you use a flask. Its the exact same sound that respeccing a passive +to stat point makes.

The bug is related to respeccing stat points specifically.

I hope I can get someone at GGG attention, this bug is really rough for a monk. I am respeccing over and over again in different ways trying to get the bug to go away.

UPDATE. If you don't have any plans of fixing this can we at least post the fix somewhere visible?

Another user figured this out. If I switch to my secondary weapon set I had 7/8 strength nodes for my Support Gems.

The "fix" is to spec strength into your secondary Passive skill points until they can allow all your Support Gems to work.

No clue why using a flask causes this but that ting sound is your support gem unequipping your Skill Gem because you don't have the nodes to hold all the support Gems

Same issue, figured out it was because of my strength support gem limit being less on my second weapon set (6) than my main (8). When I have 8 str supports using mana flask or getting hit would reset my combo and delete my placed bell. If I limit to 6 str supports then the bell works. Swapping 2 nodes str on my weapon set 2 making my str on both weapon sets equal fixed the issue.
