in the fifth act, the quest was delayed - after the murder of the traitor, the quest does not count. and also, the murder of a witch is not counted in the sixth act. As a result, you still can't get 4 passives.

I have a closed sekhem (all 4 elevations - but the quest continues to hang) and no passives have been received for it. just like with the witch in act 6, the spirit was given, but there is no pasivka. and the boss is considered not killed. In the fifth act, they say talk to Nepis (but she doesn't appear. although there are already a full backpack of these passages on sekhem) and in the sixth act, she just got carried away - and the book doesn't fall out of it.

These quests do not give passives. If you have already received 4 Ascendancy, do not pay attention to this quest. The witch icon on the map is just a visual bug.
