Simulacrum room bug

i was running simulacrum and at the stage 12/15 my character blink to other side on the map. i have autobomber bloodmage. i stuck to other side map then able to jump back to correct location but there was already tons of mobs was waiting me. and ofc instat dead. i lost my simulacrum fragment because of bug? i was collecting them 3-4 days. also ultimatum has kind a same bug. when you blink to much you can pass through gate way stone and it doesn't working any more. i lost 6 ultimatum key already. who gonna give back my simulacrum stone?
