Weapon slot stats disappearing

I have been experiencing an issue where the stats from equipped wand & focus get dropped from the current slot they are in (I am up to 4 occurrences now). I have to swap them out to the opposite slot in order for the values be added to my character (mana, energy shield, etc)

Same here, after login or die some skills disappear or are shown up in my skill bar ( staff skill sorceress, i dont want it and dropped it, after death it showed up on X Button, where i dropped it from).
