Plague Nova stops working

I'm using plaguebearer as a main clear skill, but sometimes it "overcharges" or something and stops working entirely, it is fixed by unequiping the gem, but i keep dying in higher maps when it does inevitably stop working and I dont notice it. I think it has to do with overkill damage, I'm using a wand with 3x extra damage as X with trampletoe and if I clear a big enough pack thats when the bug occurs.
