Well of Souls desecration bug

When I was in witch demon form and desecrated my item at the Well of Souls, the character did not toss it in the abyss. As a result, the npc did not come down to grab it and return it to the character. Also I couldn't use any spell, roll or turn my character around.

After I clicked on a respawn checkpoint, the game crashed, but the item was still there after restart. So it did not permanently disappear, but the desecration sequence did not work as intended.
