The Hammer of Kamasa no drop

Same here . I did the instance 3 times and the boss does not drop the hammer. On my previous character he did , not sure why it didn't this time.

Only difference I can think of is I did the Apex of Filth before Molten Vault on the character that it's not dropping for,but after on the one it worked for, not sure if that matters.
