Ancient Vows Quest Reward Duped

I finished the Ancient Vows quest and instead of giving me two different options it only gives the charm bonus but not the mana. It has both relics but both show the same reward.

I just ran into the same issue. After inserting the relics, both bonus options are the charm bonus. The flask bonus option is not available. I have used ctrl-waypoint to create a new instance as well as restarting the game. Issue persists.
