Combat Frenzy broken after death & respawn - no frenzy charge generation

After dying in any circumstances, combat frenzy stops working and there is no frenzy or power charge generation anymore UNLESS you teleport to your hideout and then back again in the instance.

Tested this in 20+ maps in various forms...
