Map is bugged in multiple ways

Maps have content that doesn't show in the map quests or on the map when you choose it. Such as alters, expeditions, and bosses. They are just in the map as I walk around.

Also, after towers are completed, the menu still shows that I can add a map, but if I try to I get a message saying it is already complete. I can't add tablets anymore.
