Omen of Amelioration bug

Still keeps happening. Again died, and it didn't activate. 3-4 days of work gone. I'm actually so pissed right now. Why does this omen even exists if it doesn't activate on death. Not returning until it activates on ALL DEATH instances. So fkn PISSED.

And that leads to another topic. Why is there no death / combat log? Not that it wasn't clear what got me this time but there's been several times I cannot tell what got me.

This morning i died from 1 shot in the 1st second of the map. I think A magic or white mob did it cause there were no rares around i mean map just started. If they give a death log .... weeeeeeelll we would see just how bugged this game is now wont we? Also 10 omens on me, now i have 9 but lost 1 full xp bar at lvl 97 . Omen activates allright ... it just does nothing . You lose xp as if u didnt have omen.

Do you happen to also use some other omen with the omen of amelioration?
For example https://poe2db.tw/us/Omen_of_Resurgence

Because you can only ever activate 1 omen in the same map instance.
If omen of resurgence has been activated in your map, then amelioration will not.

Nope. Carried only 1 omen. The one that actually matters.

Nope, one omen in my inventory. I play with convalescensce and dont need any others.
