Omen of Putrefication bugs out on L2 Withered Wand and combo stats

Upgrade a Withered Wand L2 for a noob just for fun, but...

Item: Kraken Charm Withered Wand with L2 Chaos Bolt (Item Level 4)
Quality 20%
Articifiers Orb (1 Socket made)
Omen: Omen of Putrefication
Made: by L86 Witchhunter

Application of Omen of Putrefication worked.
Revealed 2 stages and then stuck

1. 30% increased chaos damage
2. Combo:
17% increased spell damage
+17 to maximum mana
3. ----

Stuck on that part with error "crafting failed: could not generate Mod".
First it only showed "17% increased spell damage" as third option, but didn't react.
After removing and reactivating / kicking PoE2, reloading -> error above.

Another try with two wands L3 Withered / L3 Attuned Wand:
- worked as intended

After the two wands I tried again the "defect wand": it took the last option of the last wand and showed it for revealing/activating but stuck again at this point.
