Bugged Accuracy calculation for Bear? (missing melee attacks at 100% accuracy...)

Accuracy is 100% for melee so says the character sheet. Yet I'm missing 1-3 every 10 melee attacks. Is this just a hitbox thing since its alternating paws for the basic attack? Or is this a bug with accuracy. Tested this on normal non evasive mobs obviously.
