Randomly dying, no damage taken

Happening multiple times for no reason I can determine. First time was while I was interacting with Vaal Temple console, I died on the prosthetics table as well and it broke that so I couldn't use it. Captured here is the last time it happened.

Seems to be an interaction with a modified Soul Tether with the life drain mod from vaal temple. Not sure if it's intended to be able to lose a percentage of 1hp from life drain, but that's what's killing me.
