It is impossible to return to areas after death

This is what I get when I exit the map using ESC!
The sequence is as follows: I died, instead of respawning, I pressed ESC and went to the character selection screen, then I came back and ended up outside the map and got this.

P.S. There should have been 2 more portals to the map

portals depends on modifiers on waystone, if you want more portals you need less modifiers
