Portal bug/Abyss bug

Not sure if this is just console specific.
1. Noticed that I cannot directly portal of the boss arena in maps containing the 'Miller' boss. I have to walk just outside the area to portal out.
2. A couple of times, in random maps, there have been times where I can't finish the Abyss. There are no monsters around but the Abyss remains open and thus incomplete.
