Stuck at 22/24 Weapon Set Passive Skill Points after Campaign Completion

Description:
My character is missing 2 Weapon Set Passive Skill Points and is stuck at 22/24, despite completing the full campaign.

What I have completed:
• Used the Sacrificial Heart altar in Aggorat (Act 3)
• Defeated Yama the White in Halls of the Dead (Act 4), including talking to the NPC afterwards
• Completed all Interludes
• Fully exhausted all dialogue with The Hooded One in Kingsmarch
• Traveled to Oriath via the campaign completion option
• Endgame is unlocked

Expected result:
Total Weapon Set Passive Skill Points should be 24/24.

Actual result:
Character remains at 22/24 Weapon Set Passive Skill Points.
No remaining “+” icons or quest markers are visible.

Additional info:
• Issue persists after relogging and restarting the game
• Re-entering zones and re-killing Yama does not grant the missing points
• This appears to be the known 22/24 campaign completion bug
