Rewards not spawning in temple

bug id 3,126,134,383.

In my temple when I complete a room the reward is not being given.

- There's a medallion icon present but no medallion.
- There's a sealed vault with chests but no levers, there is a dead architect but no medallion or console!
- There's a historic chest but it never unlocks!

Every room with a reward!

This has happened twice in a row now, my temple seems to be broken :(
