20% is not necessary for Immortal Call and Cast When Damage Taken. You will want 20% on all other gems used by the build.

Blood Rage is optional. You can definitely use it but I find it a chore when I need to cast it every so often. :)

You can choose from the 3 skills. Personally I prefer Cyclone over the other 2 skills.

Chaos resist is optional but I would go for it since it is so much easier to obtain now. Makes mapping certain area so much easier when you have decent amount of chaos resist.
