Your gear seems fine.

Are you dieing to Guardians/Shaper/other endgame bosses? This is your bossing setup, so I assume against bosses. I am at 6.2k life and have to say the only way to avoid dieing against endgame bosses is to avoid being hit. We can't tank anything.

If you are dieing while clearing, I have no idea. I don't think I have been low life once in the last 30 T14+ maps. 90% of the mobs are dead before they come within 1 screen of me. Quill Rain + clearspeed Quiver (+1 chain quiver or a Rigwald's Quills) are a huge part of that obviously.

Have you tried Arctic Armour+Blas. Enfeeble? Has a much bigger impact than Grace for me. I haven't died once since I changed to it 4 levels ago at 86. And I changed nothing else.

And @GGZII:

For the love of god, get rid of Atziris Promise... You have Avatar of Fire. That flask does nothing. If you want the chaos res, use a Amethyst Flask with some defensive mods.

Do you think that Loreweave is better than Kaom's? I'm debating getting 6L Quill Rain & using a Kaom's, or should I get a 5L loreweave instead? Can't afford the 6L version.

5L Quill, any Chin Sol, Frostferno, Kaom's Heart

If you can't afford Frostferno yet, 5L your Chin Sol instead of QR and swap gems vs. tough bosses
