Had same problem. Went back to Uzzak or w/e. Heart DROPPED FROM RANDOM MOB after killing maybe 50-100.

Hello, I found a solution I also throw the hearth from my inventory after Act 3.
1. To see Aggorat waypoint int Act 3, go back to act 3 normal pass into time gateway, walk to the act 1 cruel town again and go straight to the waypoint
2. From the waypoint you can go through Aggorat in Act 3 normal mode
3. Go to the Sacrificial table and take the dagger
4. Even if you dont have the heath in your inventory you can put it on the table and stab it

SOLUTION: Drop sac knife on ground(can only drop it within a area not encampents). TP black chamber and walk backwards to arragot(fastest way too get back). Everything should reset knife will be at alter again and the mobs will drop the heart again. Drops from the giant mob.

worked for me i checked act 3 and just ran to alter and boom dagger was there picked it up and boom heart showed up stabbed it and got my points.
first check act 3 alter if not there then go to act 6 alter hope this helps!

Can confirm. I also dropped the heart after finishing cruel act 3, went back to aggorat and used the altar as if I had the heart in my inventory.
For the people wondering how to go back to aggorat in the past (because the portal teleports you back to the riverbank) follow this:
1- Go back to act 3 ziggurat town.
2 - Go through the time portal and be spat out onto the riverbank of cruel mode.
3- Right there near the river TP back to town.
4- You should notice it didn't take you back to Clearfell. Instead, you are now back in the old act 3 ziggurat town
5- Walk over to the east side of town. If the Town waypoint is blocked out, you're in the right zone. If not, go through the gateway and repeat steps 1 & 2.
6- Walk down the stairs and go to the drowned city and use the waypoint to tp back to ziggurat encampent (which will be in the past) then use the waypoint and tp to aggorat
*The sacrificial altar is usually located on the leftmost or rightmost side upon entering the area.
This worked for me.
