Same here. Completed. Cleared everything on the map, all rare and all boss confirmed dead. Did not get the fragment and did not get completion. It will not let me re run the citadel either

same problem here in Iron Citadel. I killed all monsters and the boss (got the fragment), but map is marked as not completed and failed.
