I died but omen of Amelioration wasn't consumed making me lose 10% xp

This happened to me as well the other day. Died 2x from a dangerous map, first time the omen worked, the second time it just didnt.

... On the other hand I prefer that GGG will care of really problematic bugs...

I had some work earlier today and then just now it has failed 3 consecutive times!

I had the same BUG on my warrior. If i use omens in that additional Bag it wont proc...If i keep it in the default one it is working....

Same issue. I have experienced the omen not consuming on death, several times over different maps. And yes it's always activated.

Take it seriously and deal with this BUG immediately.
