Abysses not opening - Patch 0.4.0d

It happened already multiple times and I thought it's known but after seeing so many hotfixes and it's still happening, I desided to report it, maybe it helps.

Seems there is an issue where monsters in the crevices aren't releasing or get stuck in the floor. Seems to happen more with the naga monsters.

" Could be but the issue is, map says there are 0 monsters left but also not sure if they are getting counted as soon as they move up. Just adding more bug reports, hopefully it will get fixed this league.Seems like the topic doesnt have tons of replies, so it will get ignored anyways since its not listed as known issues.   bug id: 1408920604 |

happened to me twice today

This started happening to me yesterday when I started doing maps at lvl 13+ on my Bear Druid. It has happened in every single map I've done since then. One of the Abyss "nodes" or whatever you call them never either provides a reward or a portal to the depths. It APPEARS to be a huge crack in the ground that I should be able to jump down into for an Abyss sub-map, but nothing. There are no enemies alive anywhere as far as I can see, and I've been randomly trying to slam the ground in case anything is stuck, but no luck.
Please fix, this is sort of ruining the whole endgame for me tbh, I love the abyss mechanic. I specifically would rather do that instead of breach or delirium, which are the weakest imo.
